How do I request an appointment, and what do I need to bring?

To schedule an appointment, please call us at 954-532-6411 or fill out the form on the right. Our appointment scheduler will respond to your request within 48 hours.

Patients should bring the following information when registering:

  • Insurance Card or Medicare/Medicaid card
  • Referral forms from your physician (if required by your insurance carrier)
  • Referral/Order Guidelines
  • Driver’s license

What happens after my surgery is scheduled at Broward Outpatient?

Our staff will contact you before your surgery to prepare you for surgery, and let you know the things you need to do. If you have not been contact by the day before your surgery, please call 954-532-6411 and ask to speak to someone regarding your "surgery preparation."  You doctor will also likely give you preparation materials at the medical assessment scheduling the surgery - it is important to follow instructions. 

There may be special instructions regarding whether you can eat or drink prior to the surgery.  Also, it is important to be clean before the surgery: be sure take a good shower to ensure the surface of your skin is not carrying any additional bacteria that can cause infections. 

On the day surgery, wear loose-fitting, comfortable clothes and slip-on flat shoes. 

How early do I need to check in on the day of my surgery?

Please check in to the medical center 1.5 to 2 hours prior to your scheduled surgery time, unless specifically directed otherwise, or you are there for pain management. Pain management patients are required to check in one hour prior to the scheduled time.

It is rare, though possible that your surgery time can change. For this reason, it is imperative to have the correct contact information in your patient file. 

Do you file my insurance claim or do I?

At Broward Outpatient Medical Center, our billing staff is able to file with most major medical insurances. If you have questions regarding your coverage, please call your insurance company just to make sure. If you have already had a medical visit at our facility, you can call our billing department to ask about the status of your claim or to discuss any payments due.

Is my healthcare information private?

Yes. Keeping medical information private is the law. At Broward Outpatient Medical Center, we follow all HIPAA privacy and security guidelines and our staff is fully trained on the importance of confidentiality. If you have specific questions about who has access and how, please call our Patient Advocacy Team at 954-532-6411.
